Arthur found his permanent home at Community Link House supportive housing
When Arthur moved in with his grandson, he thought life would be easier. His wife passed away and with his declining mental and physical health, having family to support him sounded like the perfect plan. Unfortunately, Arthur fell victim to senior abuse and found himself with nowhere to live.
Further health complications landed him in the hospital. While the care he received was excellent, Arthur was worried about where he would go after being discharged. Fortunately, staff referred him to Transitional Care at The Neighbourhood Group. This service provides supportive housing for people discharged from hospitals but are not ready to return home, or are waiting for space in a long-term care home.
For Arthur, the transitional apartment at the Cecilia Murphy building was a blessing. Personal support workers were on hand to provide daily check-ins, medication monitoring, as well as assistance with homemaking, laundry and other daily activities. And Arthur took advantage of Meals on Wheels, on-site recreation, and case management to help coordinate and navigate the services he needs.

I get the same services I did in Transitional Care, but now I know I have a permanent home I can afford. Living here made a huge difference in my daily life, and I'm truly thankful for their help."
