The Administrative Lead, MSSP provides admin support for our Mobile Shelter Support Program (MSSP) and other mobile community health programs.   • Community Mediation Worker - Contract
    Our Community Mediation Services is a free, confidential and voluntary community mediation service that supports individuals, groups and communities during conflict and provides a collaborative approach to conflict resolution. Our service provides volunteer mediators who facilitate discussions between parties, assisting them to reach a mutually agreeable solution. The Case Worker supports the operations of our community mediation and restorative justice services, in collaboration with the Team Lead, volunteer mediators, and student and interns.     The Controller reports directly to the CFO at the Neighbourhood Group Community Services (TNGCS) and is responsible for managing all financial operations including overseeing accounting processes, payroll & benefits, internal controls and financial reporting activities. The Controller works closely with the CFO to respond to concerns from the senior leadership team to ensure that program areas meet financial reporting requirements set out by Funders. The incumbent will also produce all financial reporting that goes to the Board.   • Corner Drop-In Relief
    The Corner Drop-in is a busy, dynamic program that serves ~ 200 participants each day. We provide daytime shelter, nutritious hot food, primary health care services from on-site nurses, doctors and psychiatrists, information and help finding affordable housing, mental health support, voluntary financial trusteeship, substance use counselling, workshops and activities. Relief staff will work as a member of the Drop-In team to provide an enriched drop-in setting for homeless and socially isolated adults. Working alongside staff, volunteers and peer workers, relief staff will ensure a safe and welcoming environment. This position works on an on-call, as-needed basis to replace door and/or floor staff in drop in. Relief staff will help maintain order and safety, participate in crisis intervention, assist drop-in members with accessing services, and make referrals.

The Doorperson will staff the front door during the hours the Drop-in and Trustee programs are open and monitor the building activity.

  • Follow Up Worker - Stabilization & Connection Centre
    As a member of the Urban Health and Homelessness Services Department, this position provides direct support to individuals discharged from Toronto’s first Stabilization Centre, serving intoxicated vulnerable adults diverted from the community by Emergency Services/paramedics. Working from a harm reduction, trauma-informed perspective, the Follow-Up Worker, Stabilization Site, assists participants in implementing their discharge plans through the provision of aftercare via warm referrals, accompaniment, and other one-on-one short team supports.     As part of the Integrated Employment Services team, the Job Developer – Integrated Employment Services plays a pivotal role in connecting job seekers, with a focus on Stream C clients with meaningful employment opportunities.   • Mobile Overdose Prevention Worker - Part-Time - Contact (2 Positions)
    The Homelessness and Addiction Recovery Treatment (HART) Hub demonstration project is a multi-disciplinary, collaborative initiative between the Parkdale Queen West Community Health Centre, and 11 partnered organizations. Through a “hub and spoke” model we will provide culturally safe and trauma informed services to individuals with complex health and social care needs. Services will include primary care and substance use care (continuum of treatment, recovery and harm reduction); mental health care and support services; supportive housing including shelter and transition beds; and social services including peer support, case management, and employment supports. Urban Health and Homelessness Services of The Neighbourhood Group (TNG) is looking for experienced individuals with lived and living experience of drug use for our Mobile Overdose Prevention Project. The Mobile OD Prevention Workers will engage with people in our community who are experiencing homelessness, particularly those who are at risk for overdose, to assess their needs and connect them to services, including housing, ID services, income supports, etc. Working as part of the UHHS team, and with community partners, the Mobile OD Prevention Workers will build trust with people in our community, respond to overdoses, ensure basic needs are met, and refer them to further supports.

  • Mobile Shelter Support Program Case Manager
    As a member of the UHHS department, this position provides direct case management supports to adults living in Covid-19 shelter recovery/isolation hotels as well as clients living in different shelter and respites around the GTA. This support will serve clients with a complete range of complex issues, including health, financial, housing, legal and life skills issues. Working from a harm reduction, trauma-informed perspective, case managers provide extensive, direct one-to-one support, on-site visits and telephone contact to assist participants in identifying and achieving self-determined goals. Working closely with the client and external service providers involved, the Case Manager liaises with and coordinates access to services including substance use and mental health supports, landlords, health care providers, legal institutions, income supports, housing providers and other community services.

  • PAID Mobile Identification Outreach Worker
    Mobile ID Services for Encampments is an initiative under the Partners for Access and Identification (PAID) Project to assist the Rapid Housing Team of Shelter Support and Housing Administration in quickly housing individuals living in encampments across the GTA. The mobile PAID worker will work alongside Streets to Homes and other outreach staff to connect with those in need of identification to finalize relocation and housing access.     The Neighbourhood Group (TNG) is partnering with the University Health Network (UHN) to support vulnerable adults diverted from UHN Emergency Services. Operating from a harm reduction and trauma-informed perspective, the Supervisor, Winter Support Hub (WSH) provides program leadership with 1 Shift Lead and a team of Peer Support Workers, as part of an interdisciplinary team, including medical professionals, community partners, health navigators, and harm reduction workers. The Supervisor ensures that the team assists clients in building rapport, navigating services, de-escalating conflicts and supporting the development of care and discharge plans. The Supervisor works alongside the team during operational hours (7:30 PM to 8:30 AM, weekdays and weekends), ensuring continuous support, coordination, and oversight. Operating from a harm reduction and trauma-informed perspective, the Team Lead assists the Supervisor in providing program leadership with a team of Peer Support Workers, as part of an interdisciplinary team, including medical professionals, community partners, health navigators, and harm reduction workers. Leveraging their own personal experience with homelessness, mental health challenges, and substance use, the Team Lead works in close collaboration with the peer team in their work assisting clients in building rapport, navigating services, de-escalating conflicts and supporting the development of care and discharge plans. The Team Lead works alongside the team during operational hours (7:30 PM to 8:30 AM, weekdays and weekends), ensuring continuous support and coordination. This position reports to the Supervisor, Winter Support Hub. The Team Lead, Winter Support Hub will be required to work evenings, weekends, statutory holidays, and overnights.

  • Transitional Case Manager - Contract
    The Transitional Case Manager is responsible for a caseload of seniors, supportive housing clients and/or adults with complex medical needs who will require a high level of support in several areas while living in transitional housing units in the Greater Toronto Area. The position’s main duties include assessment, intervention, advocacy, supportive counselling, financial counselling and ongoing case management. Hours of work will be 35 per week, and may include evenings and weekends.

    This position provides services to youth who have come into contact with the youth criminal justice system, providing intake, support, and the implementation of appropriate sanctions and measures, and the fulfilment of legislated reporting requirements. The worker will facilitate court sanctioned or mandated workshops, providing advocacy and/or court supports to youth/parents and supporting wider violence prevention initiatives. They will work within a dynamic team of youth justice workers to decrease violent crime among youth and support those youth at most risk of violence. The position delivers programs internally and within community-based settings that value youth leadership, youth empowerment and self-determined care and development.
