Transitional Care Program

Seniors who are waiting in the hospital for a long-term care home or who need more time before returning home require extra help. Our 24-7 intensive Transitional Care Program does just that. By offering services like personal support workers, case management and emergency response in a temporary supportive apartment, seniors can safely make the transition back home or to a new long-term care residence.

Eligibility

Age 59+ and in the hospital waiting for long-term care placement or who need more time before going home

Length of stay

3 to 6 months on average

Registration

Referrals are made to Bellwoods by social workers in the hospital or Home and Community Care Support Services

Residents in our Transitional Care apartments receive:

Fully-furnished apartments with basic cable TV and home phone
24-hour personal care, medication monitoring, meal preparation, light housecleaning
Emergency lifeline system
Intensive case management and care coordination

FAQs

What happens after I am ready to leave the Transitional Care apartment?
Once you are ready, you can go back home or to a long-term care home. Our Case Management team will help you with whatever you need and will make sure you have services to help you at home.
Do you have to pay rent for the unit?
Not at all. The rent and personal support services are free.
